(1) If any member of the public loses a properly completed form issued, and applies for a duplicate, a certified true copy or a photocopy may be given and a new printed form shall not be used except where this is provided for by law,  and the appropriate fee has been collected

(2) Departmental Accounting Instructions may specify a fee to be collected for the provision of copies.
