(1) A person who desires to engage in the business of a customs house agent shall apply to the Commissioner-General in writing or electronically in a form determined by the Commissioner-General.
(2) The application shall include the following particulars:
(a) the business name and address of the applicant;
(b) the place of business of the applicant;
(c) the certificate of incorporation of the business or proof of registration of partnership;
(d) the Customs Proficiency Certificate; and
(e) the Tax Identification Number of the applicant.
